Parish Event Communication Form

This form should be used to communicate with the church/rectory office about your event and to specify how you would like it to be published. 

This form should be submitted two (2) weeks before the day you want your event published.

Submitting this form does not guarantee publication. (The Parish Administrator must approve all events and information before publication.)

ACH Debit Form

  • Authorization Agreement for ACH Debits (Can also be used to change your debit amount.)
  • Seal the completed form in an envelope with 'FOR OFFICE' written on the front and drop it in the Sunday Collection.  You can also drop it off at the rectory during office hours or use the mail slot in the door.
